hi, i'm dr. young seattle facial plastic surgeon, we're doing chin augmentation. i just wanted to explain a little bit about the process. so far right here we've done an incision underneath the chin. this persons' chin. and developed a pocket all the way out to here. and all the way out to hereon the inferior border. as you can see... there is an implant that is already in. in order to prevent dead space from accumulating. and empty fluid accumulating we put a screw here. to tack down, you can see the screw there. if you get in closer you can see the screw. the screw is there to hold the implant in place. and while i'm doing this.

i can make sure that it's in the right position along the inferior border. by looking in here. i see that its just right and i'm going to do it on this side. we'll look inside to see if it's just right. and it looks pretty even on both sides. and i also want to make sure that this is in the midline. there is a little raphe here that separates the two implants and that allows us to put it in. this is the reason why i went under the person's chin. because this is the most... although this is an internal incision. its pretty much the way that decreases the complication rate of chin implants. it all has to do with if you go through the mouth which is the other main option. you go through the muscles that control the lip and chin movement.

and that can create a lot of deformities in the chin. there is a recent study that showed that most of the complications that occur... with a chin implant was going through the mouth approach. so i never go that way. in addition when i go through the mouth approach. i contaminate the implant. which you don't do as much when you go through the skin incision. so right here we're basically using the implant to project the chin forward for a recessed chin. and we can see the outline there. this is a medpor implant. as you can see as you get really close you can see that they have these fine little fine little particles. now these are not really fine little particles but perforations that allow...

blood vessels to grow in there or to heal in there to avoid infections. large enough to allow blood vessels to grow in there but not too small... so that the bacteria can hide in there. at 12 weeks this starts to act like your own tissue. and so this is the bottom of the chin. you definitely want the implant level... with the bottom of the chin. in her case i didn't want to make her chin any longer. so i didn't want the chin implant to ride too inferiorly along the bottom of the chin. i wanted to keep it on the anterior face of the chin. to control the distance of her, the length of her lower third. from her lower lip to the bottom of the chin. so right now we're going to close the incision.

on this side you can also see the screw right there. located in this area. so the next step is to close it. essentially we're pretty much done. thank you!
